What is Chronic Care Management (CCM)?
Chronic Care Management (CCM) is a service that helps you manage your health if you have two or more chronic conditions expected to last for more than a year. CCM can help you improve your quality of life and prevent complications from your conditions. It focuses on creating a team-based, patient-centered approach designed to assist patients and their support systems in managing their chronic medical conditions more effectively.
What is a Chronic Condition?
Examples of chronic conditions include, but are not limited to:
- Alzheimer’s disease and related dementia
- Arthritis (osteoarthritis and rheumatoid)
- Asthma
- Atrial Fibrillation
- Autism Spectrum Disorders
- Cancer
- Cardiovascular Disease
- COPD
- Depression
- Diabetes
- Hypertension
- Infectious Diseases such as HIV/AIDS
- Substance Use Disorders
There are many other conditions that can qualify you for CCM. Based on your condition(s), your doctor or medical staff should know whether or not you qualify for CCM services.
How Does CCM Work?
CCM is provided by a team of healthcare professionals who work with you and your primary care provider. They will create a personalized Comprehensive Care Plan that covers your goals, needs, and preferences. They will also coordinate your care with other providers and services, such as specialists, pharmacies, labs, and community resources. You can contact your CCM Nurse anytime you have questions or concerns about your health.

The overall goal of CCM is to keep you healthy while reducing costs. Better care management helps you avoid health events such as trips to the emergency department, falls, or worsening health. CCM allows you to get the personal attention of a registered nurse and help from a healthcare provider you know, and who knows your health conditions. How Can I Get CCM?
CCM is covered by Medicare Part B if you have two or more chronic conditions that are expected to last at least 12 months or until the end of your life.
